Settlement include all U.S. residents who purchased the covered TripleFlex products (NatureMade-brand TripleFlex supplements) since May 13, 2007 and the preliminary approval date. Who submit timely and valid Claim Forms are entitled to receive a cash award (for up to four bottles of the product) and/or a product award (good for up to six bottles of offered products). Class Members who submit proof of purchase with their Claim Forms can request a payment of $25 per bottle and Class Members who do not submit proof of purchase may request $12.50 per bottle.
